  2. Form 2020-U: If you need transportation outside the Common Medical Marketing Area (CMMA), this form is required.
  3. Personal information: Name, date of birth, address, contact number, reason for transportation, date and time of the medical appointment, appointment location, and any special instructions.
  4. Initial contact: Speak with your primary care provider or case manager to begin the process.

For transportation providers:

  1. Insurance: You must provide proof of general liability insurance, workers’ compensation coverage, and disability coverage.
  2. Licenses and registrations:
    • Driver licenses verified through the NYS DMV LENS program.
    • Vehicle registration and insurance, including VIN numbers.
  3. Training: Drivers must complete the PASS (Passenger Assistance, Safety, and Sensitivity) program within the first three months after implementation in MAS.
  4. GPS: Accurate GPS information must be recorded and provided for each trip.
  5. Additional documents: Forms such as C-105.2, U-26.3, and DB-120.1 may be required to upload to the MAS portal.
